  • Trial ends automatically after 14 days
  • No charges unless you subscribe

We recommend using the free trial to ensure FormAlert meets your needs before subscribing.

Subscription Cancellation

How to Cancel

You can cancel your subscription at any time:

  • Log into your FormAlert account
  • Go to Settings → Subscription
  • Click "Cancel Subscription"
  • Confirm cancellation

What Happens When You Cancel

  • Your subscription will not renew
  • You keep access until the end of your current billing period
  • Your data is retained for 30 days after cancellation
  • You can reactivate within 30 days without data loss
  • After 30 days, your data is permanently deleted

Refund Eligibility

7-Day Money-Back Guarantee

We offer a 7-day money-back guarantee on your first subscription payment:

  • Applies only to your first paid subscription (after free trial)
  • Request a refund within 7 days of your first payment
  • Full refund, no questions asked
  • Only applies if you used FormAlert for legitimate business purposes

Service Failure Refunds

We may issue refunds at our discretion if:

  • FormAlert experiences extended downtime (more than 48 hours)
  • Critical features are unavailable due to our error
  • You were charged incorrectly due to a billing error
  • We fail to deliver the service as described

Non-Refundable Situations

Refunds are not available in the following cases:

  • Partial months: We don't issue prorated refunds for cancelled subscriptions
  • After 7 days: Beyond the 7-day guarantee period for first subscription
  • Renewal payments: Monthly renewals are non-refundable
  • Change of mind: After using the service beyond the guarantee period
  • Account violations: If your account was terminated for violating Terms
  • Third-party fees: Payment processor fees (Razorpay) are non-refundable
  • Free plan: No refunds on free plan (nothing paid)

Disputed Charges

If you notice an unauthorized or incorrect charge:

  • Provide transaction details and reason for dispute
  • We'll investigate and respond within 2 business days
  • We'll issue a refund if the charge was made in error

Plan Upgrades and Downgrades

Upgrading Your Plan

  • Upgrade takes effect immediately
  • You're charged the prorated difference for the remainder of the billing cycle
  • Next billing will be at the new plan rate

Downgrading Your Plan

  • Downgrade takes effect at the end of current billing period
  • You keep Premium features until your plan renews
  • No refund for the difference in plan pricing

Special Circumstances

We understand that special situations may arise. We may consider refund requests outside our standard policy for:

  • Medical emergencies or serious illness
  • Death of the account holder
  • Natural disasters affecting your practice
  • Other extraordinary circumstances
