The problem
Appointments are being taken in four different places and none of them agree with each other.
The fix
This step creates the single place they'll all land. It gives your clinic a booking page of its own — a web address you can put in a WhatsApp reply, on Google, or on a poster at reception — and sets the rules every booking follows, whichever door it came through. Everything else in FormAlert hangs off what you fill in here, so it goes first.
Your Google Business Profile. FormAlert finds your clinic on Google and pulls in your name, phone number, description, category and services, so you type almost nothing. It's also where your review link comes from. No listing yet? Create one free at business.google.com first — it takes ten minutes and you need it either way.
A WhatsApp-active phone number. Every booking alert lands here, and it's the number patients are told to call.

Go to formalert.in and click Get Started Free.
Sign up with Google, or with an email address and password.

Two screens. The first finds you on Google, the second lets you correct whatever it got wrong.
Find your clinic. Search for your business the way it appears on Google and pick it from the results. FormAlert pulls in your clinic name, WhatsApp number, description, category and service list automatically.
Confirm your details. Review everything that came across and edit anything wrong — clinic name, WhatsApp number, the doctor patients will book with, the category you want to rank for, your description, and your starting list of services. Then click Complete Setup.
Go to Settings in the sidebar. The left column is your clinic.
Still in Settings → Business profile, set your Booking URL slug. This is the part of the link patients see, read out over the phone, and type.
Type the slug you want, e.g. dr-sharma or city-care-clinic.
FormAlert checks it live and tells you whether it's free or taken.
Save. Your booking page is now formalert.in/book/dr-sharma.
In the right column of Settings, under Scheduling. These decide what patients can and can't do to your calendar without asking.
Under Settings → Patient intake you can ask questions during booking — reason for visit, existing conditions, whatever you'd otherwise ask at the desk.
Click to add a question and type it.
Choose the answer type: Short answer or Dropdown.
For a dropdown, enter the options patients can pick from.
Save. The questions appear on your booking page immediately.
Under Appearance, set your booking page colours. On the Business plan you can also upload a hero image — a wide landscape banner across the top of the page.
On the Dashboard, the Close button stops new bookings without touching your schedule. You can set a date to stay closed until, and the button flips to Open to bring bookings back.
My booking page loads but shows no slots at all.
Expected at this stage — slots come from services, and you haven't added any yet. That's step 2, Team Schedule.
The wizard couldn't find my clinic on Google.
Either you have no Google Business Profile, or its name differs from what you typed. Search the exact name shown on Google Maps. If there's genuinely no listing, create one at business.google.com — you'll want it for reviews regardless.
Booking alerts aren't reaching my phone.
The number in Settings → Business profile is probably the landline Google had on file. Replace it with a number that has WhatsApp active on it.
The slug I want is taken.
