How to Use FormAlert
Step-by-step guides for every part of the software.
Phase 03 · Providers & Services
Add doctors, configure services and schedules.
Add at least one provider to start taking bookings. Each provider has their own services and schedule. Google Calendar sync is required for online consultations (to auto-generate Google Meet links).
Adding a provider

Name & photo. Shown on the booking page when patients choose a doctor.

Services offered. Each provider can offer a different subset of your clinic's services.

Service type. Offline (in-person) or Online (Google Meet auto-generated). Online requires Google Calendar connected.

Payment type. Prepaid (patient pays at booking via Razorpay/UPI) or Postpaid (pays at the clinic).

Duration & timeslots. Set a service duration (e.g. 30 min). FormAlert automatically generates all bookable slots within your availability hours.
Team roles
Each team member has a role that controls what they can see and do in FormAlert.
Admin
- Full access to all features
- Can add and remove team members
- Can access billing and plan settings
- Can view all appointments and patients
- Can edit clinic settings and services
Doctor
- Can view and confirm appointments assigned to them
- Can view the patient list
- Can manage their own schedule
- Cannot access billing or team settings
Staff
- Can view and create appointments
- Cannot access billing or settings
- Typically used for reception desk staff
Adding a team member

Go to Team in the main navigation.

Click Add team member.

Enter their name, email address, and choose their role.

Click Send Invite.

They receive an email with instructions to set a password and log in.
Removing a team member: go to Team, find the person, and click Remove. Their past appointments and activity are not deleted.
Tip: Google Calendar sync must be connected per provider if you want to offer online (Google Meet) consultations or prevent double-bookings. Go to Team, open the provider, and connect their Google account.
