Phase 03 · Providers & Services
Add at least one provider to start taking bookings. Each provider has their own services and schedule. Google Calendar sync is required for online consultations (to auto-generate Google Meet links).
Go to Team in the sidebar and click Add Provider.
Enter the provider's full name and designation (e.g. "Dr. Riya Kapoor, Dermatologist").
Optionally add a bio and contact email for the provider.
Click Save Provider. The provider now appears in the list and on the booking page.
Open the provider from the Team list and click Add Service.
Enter the service name (e.g. "General Consultation") and fee in rupees. Set the fee to ₹0 if the service should be free.
Set the duration in minutes — FormAlert divides your availability hours into slots of this length automatically.
Choose Service Type: Offline (in-person at the clinic) or Online (Google Meet link auto-generated — requires Google Calendar connected).
Choose Payment Type: Prepaid (patient pays at the time of booking via Razorpay or UPI) or Postpaid (patient pays at the clinic, no upfront payment screen).
Click Save Service. The service appears on the booking page immediately.
On the provider's service row, click Set Schedule.
Toggle each day of the week on or off.
For each active day, set a start time and end time (e.g. 09:00 – 18:00).
Add multiple time blocks per day if needed — for example 09:00–13:00 and 15:00–18:00 to block a lunch break.
Click Save. The booking page updates immediately with the new available slots.
Open the provider from the Team list.
Toggle Mark as Unavailable and set the date until which they are unavailable.
Click Save. The provider's slots are hidden from the booking page until that date.
To restore availability, toggle the setting off and save again.
Open the provider from the Team list.
Click Connect Google Calendar.
Sign in with the provider's Google account and allow access.
Blocked time in Google Calendar (meetings, personal events, lunch breaks) now appears as unavailable on the booking page automatically.
Each team member has a role that controls what they can see and do in FormAlert.
Go to Team in the sidebar.
Click Add Team Member.
Enter their name, email address, and choose their role (Admin, Doctor, or Staff).
Click Send Invite.
They receive an email with instructions to set a password and log in.
